More than half of California's Medicare beneficiaries are enrolled in Medicare Advantage plans — and that number keeps growing. In most California counties, you can get dental, vision, fitness benefits, and sometimes grocery allowances bundled into a $0 monthly premium plan. On paper, it sounds like a no-brainer. But Medicare Advantage is more nuanced than the ads make it seem.
What Is Medicare Advantage?
Medicare Advantage (Part C) is a private insurance alternative to Original Medicare. Instead of the federal government paying your doctors directly, a Medicare-approved private insurer manages your coverage — bundling Parts A, B, and usually D into one plan.
You must have Part A and Part B to enroll, and you continue paying your Part B premium ($202.90/month in 2026) regardless of which Advantage plan you choose.
The California Market in 2026
California has 402 Medicare Advantage plans available in 2026, with over 3.6 million beneficiaries enrolled (51% market penetration). Average statewide premium: $11/month. Most $0-premium plans are widely available in urban counties.
Top carriers by J.D. Power member satisfaction (2026):
- Kaiser Permanente: 675/1000, 4.5 Star CMS rating — largest single plan in CA with 180,321 LA County enrollees
- SCAN Health Plan: 672/1000 — California nonprofit, strong community-based care model
- Alignment Health Plan: 658/1000 — tech-forward, innovative plan variants including grocery allowances
- UnitedHealthcare: widely available across all major California counties
- Humana: 3.5 Star rating, strong supplemental benefits
HMO vs PPO — The Plan Type Decision
This is the most important Medicare Advantage decision — more important than which carrier.
| Plan Type | How It Works | Best For |
|---|---|---|
| HMO | Choose a primary care doctor who coordinates care. Specialists require referrals. Out-of-network means paying full cost. Average LA County HMO premium: $2.42/mo (48 of 52 HMO plans are $0). | People with consistent healthcare needs and established in-network relationships |
| PPO | See any Medicare-accepting provider. Out-of-network costs more, but you're still covered. Average LA PPO premium: $17.25/mo. | People who travel, want flexibility, or have doctors they're unwilling to leave |
The Most Important Thing to Check: Your Doctors
Before you enroll in any Medicare Advantage plan, check the provider directory. Confirm your primary care doctor, the specialists you see regularly, and your preferred hospital are all in-network for that specific plan.
Search our California doctor directory to verify whether your physicians accept Medicare — and whether they're in-network for specific Advantage plans — before you enroll.
Avoid surprise out-of-network costs by checking first.
Extra Benefits — What's Real and What to Watch
Advantage plans can offer what Original Medicare doesn't: dental, vision, hearing, gym memberships, OTC allowances, transportation, and more. Evaluate them — but don't let a $50 grocery allowance distract you from checking whether your cardiologist is in-network.
- Dental: $500–$2,000 annual benefit for cleanings, x-rays, basic restorative work
- Vision: annual eye exam + $100–$200 eyewear allowance
- Fitness: SilverSneakers or Silver&Fit memberships on many $0 plans
- OTC card: $25–$100/month for over-the-counter health products
Core medical coverage first. Extra benefits are the bonus.
Meet Patricia From Torrance
Patricia — Torrance, CA
Patricia was on Original Medicare for three years, paying separately for dental and a Medigap supplement — over $350/month combined. During AEP, she learned that a Kaiser Senior Advantage plan in her area cost $0, included her primary care doctor, covered basic dental and vision, and had a $3,400 in-network out-of-pocket max.
She switched. Her Medicare costs dropped by over $200/month. The key: someone helped her compare the total picture, not just the premium headline.
When You Can Enroll

- Medicare Advantage Open Enrollment (OEP): January 1 – March 31 — switch Advantage plans or return to Original Medicare
